BIRTH REGISTRATION

If you are an Italian Cityzen and had a child in Japan, you must register it’s birth to Italian authorities as well, even if you hold other citizenships.

In order to do so, you need to send to the Consulate the following documents:

  • Application form (download here), signed by both spouses
  • Copy of both parents’ passports
  • Birth Certificate (shusseitodoke jurishomeisho 出生届受理証明書), apostilled by Japanese Ministry of Foreign Affairs (Information here: http://www.mofa.go.jp/ca/cs/page22e_000416.html)
  • (in case one of the spouses is a Japanese Citizen) family register (koseki tohon – zenbu jikoshomei 戸籍謄本全部事項証明), also apostilled by Japanese Ministry of Foreign Affairs (Information here: http://www.mofa.go.jp/ca/cs/page22e_000416.html)
  • If the child was born with mother’s surname only and you wish it to have father’s surname (either added to or instead of mother’s surname) you will need to submit a further application (download here). In this case application need to be done in person.
